Identifying Patients With Secondary Mitral Regurgitation and Heart Failure who may Benefit From Transcatheter Edge-to-Edge Repair in the Wellington Region

As the New Zealand population ages, incidence of heart failure and valvular disease increases. The Cardiovascular Outcomes Assessment MitraClip Percutaneous Therapy for Heart Failure Patients with Functional Mitral Regurgitation (COAPT) trial showed decreased rates hospitalisation all-cause mortality transcatheter edge-to-edge repair (TEER) clinically significant secondary mitral regurgitation (MR) within 24 months follow-up compared medical therapy. This retrospective observational study aimed to identify patients who may benefit from TEER in Wellington. retrospectively reviewed electronic records 337 consecutive aged ≥18 years presented Wellington Hospital April September 2022 identified those met modified COAPT criteria: at least moderate regurgitation, left ventricular ejection fraction (LVEF) 20%–50% on echocardiography, did not meet criteria cardiac surgery or other valve intervention. Forty 27 (68%) were male 13 (33%) female. mean age was 71.7 years. Of them, 55% European, 23% Pasifika, 13% Māori. A total 58% had MR, 33% moderate–severe 10% severe MR; median LVEF 30%. survival 309 days 365 (non-statistically significant). Among failure, a proportion (11.9%) trend towards shorter times present general. According international guidelines data such as COAPT, these would have potentially benefitted TEER.
